CFI Awarded DHS Contract to Continue Targeted Case Management Services in Milwaukee County

Centers for Independence (CFI) has been awarded the Targeted Case Management (TCM) contract through the Wisconsin Department of Health Services (DHS). This achievement continues our long-standing partnership with DHS and reinforces our role as one of Milwaukee County’s largest providers of vital behavioral health services.

About TCM

Targeted Case Management is often the first step for adults entering the behavioral health system. Through TCM, CFI assesses individual needs, connects clients to community resources, and provides ongoing monitoring to support recovery and stability. These services help forge connections with essential providers such as doctors, therapists, housing supports and food pantries, creating a strong foundation for long-term wellness.

The Impact

For more than 30 years, CFI has been a trusted partner in Milwaukee County’s behavioral health continuum. The TCM program helps individuals navigate complex systems, avoid crises, and access housing, health care and social supports.

With this contract, CFI will continue serving about 250 clients annually, addressing critical gaps in care for vulnerable populations. This work reflects our unwavering commitment to equity and quality. This milestone reinforces CFI’s mission to serve Milwaukee County with compassion and excellence—a testament to our shared dedication to improving lives and building healthier communities.
