In this role, you’ll provide comprehensive clinical care with accuracy and compassion. You’ll carry out clinical skills to meet standards of care, make ongoing patient assessments and revise treatment plans in collaboration with physicians. Case Manager
Is mental health important to you? Do you want to help adults with their mental health? We’re looking for case managers to provide personalized, client-centered care to adults with mental illness. Working collaboratively with clients, you’ll identify strengths, needs, goals and a life vision, then develop and implement treatment plans.

You’ll connect clients to behavioral health providers, monitor medications, access AODA services, develop life skills and link clients to resources.
